Billing

Warde Medical Laboratory offers a varied billing program to help meet our client’s needs.  The client account will be billed directly for all testing unless arrangements to bill third parties have been approved by our Finance Department.  If approved, clients may choose either Account or Third Party/Patient billing.  When choosing Third Party/Patient billing all required information must be enclosed with the test requisition.  Unless all the third party or patient billing information is provided, test charges will be billed to our client’s account directly.

Due to federal legislation prohibiting the practice, we cannot accommodate client requests for courtesy service. Physicians and staff can have tests charged either to their account or to their insurance. Once the insurance choice is made, any fees not covered by the insurance company cannot be billed to an account bill. Physicians and staff will be required to pay all co-payments and deductibles owed to the insurance company, as would any other patient.

Test Classification

  1. FDA approved (IVD)
  2. FDA exempt (in-house developed assays)
  3. Analyte Specific Reagent (ASR)
  4. Research Use Only (RUO)

Where appropriate, the laboratory report will contain a statement regarding the classification, test development, and performance characteristics of individual assays.  

Account Billing

Clients will be billed on a monthly basis. The itemized invoice will include the date of service, patient name, medical record number, test name, test number, CPT code, and fee. A change from account to third party/patient billing may be requested within 30 days of the invoice date. After the 30 days, no adjustments will be made and the client will be responsible for the invoice amount. Invoice payments are due to be paid within 30 days of the invoice. All past due invoices are subject to a 1.5% interest charge per month. A late payment charge will be applied to the account, and will continue to accrue until the invoice is paid. Non-contract fees are subject to change without notice. Questions regarding the invoice may be directed to the billing department at 800/876-6522.

Third Party/Patient Billing

Warde Medical Laboratory is a provider for several insurance companies, HMO's, PPO's, Medicare and Medicaid. We will bill insurance companies directly and accept assignment of payment when we have complete and accurate billing information. When requesting direct patient billing, the responsible party must be listed for patients under 18 years of age. Fees charged for third party and patient billing are in accordance with the usual and customary fees charged by the medical community, however they are subject to change without notice. A listing of these fees and CPT codes will be provided upon request.
